Besides addressing the health needs of the population and their patients, they also have the task of <\/span><b>intercepting and preventing such needs <\/b><span style=\"font-weight: 400;\">from emerging. They do this by collaborating with all the resources available inside the community: families, volunteer associations, parishes, and neighbors.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">They have specialized training in clinical care and management. They have expertise in the field of primary care, prevention, public health, care management and health monitoring. Finally, they use a patient-centered approach that requires empathy and interpersonal skills.<\/span><\/p>\n<p>&nbsp;<\/p>\n<h4>What do they do?<\/h4>\n<p><span style=\"font-weight: 400;\">The IFeC performs various tasks inside a specific geographical area and target community. They provide social and health assistance at educational establishments and care residences for adults who are not self-sufficient. Besides this, they have a role of particular importance in the home assistance of <\/span><b>older persons, children, <\/b><span style=\"font-weight: 400;\">and <\/span><b>adults who are not self-sufficient, the chronically ill, vulnerable individuals<\/b><span style=\"font-weight: 400;\"> and <\/span><b>persons with disabilities<\/b><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h4>With whom?<\/h4>\n<p><span style=\"font-weight: 400;\">The IFeC works in constant dialogue <\/span><b>with all the social and health professionals and institutions that surround the patient<\/b><span style=\"font-weight: 400;\">: general practitioners, pediatricians, specialist doctors, social services, social and healthcare professionals, social welfare workers, the emergency medicine services, hospitals, clinics and various Case della Salute. Furthermore, they <\/span><b>understand the older person\u2019s environment and daily life<\/b><span style=\"font-weight: 400;\">, such as associations, groups, and social clubs. They support communication and connection between all actors of the social and healthcare network, in order to <\/span><b>facilitate integrated and synergistic care <\/b><span style=\"font-weight: 400;\">in a multi-professional environment.<\/span><\/p>\n<p>[\/et_pb_accordion_item][et_pb_accordion_item title=&#8221;IFeC, what are the regulatory references?&#8221; _builder_version=&#8221;4.9.2&#8243; _module_preset=&#8221;default&#8221; open=&#8221;off&#8221;]<\/p>\n<h4>Italy<\/h4>\n<p>Introduced and described by the World Health Organization in 2000, the role of the IFeC was officially established in Italy with the 2019-2021 Health Pact, approved by the State-Regions Conference on December 18, 2019. This pact represented a 3-year financial and program agreement intended to improve service quality, continued assistance, and the integrity of the National Health Service.<\/p>\n<p>Prior to that, the IFeC system was already active in some ASLs of certain Regions, like Friuli-Venezia Giulia and Tuscany, but without any clear definition of the role and, most importantly, without formal recognition at the national level.<\/p>\n<p>On May 13, 2020, the Relaunch Decree introduced the role of the Family and Community Health Nurse to increase the care of persons in the area infected by SARS-CoV-2. The provisions set out by the Decree took effect two months after the passage of Law 77\/2020.<\/p>\n<p>On September 10, 2020, the Conference of the Regions approved the Family\/Community Health Nurse Guidelines Ex l. July 17, 2020 n. 77, a document that standardizes the IFeC figure in diverse regions, defining its responsibilities, contractual framework, and training. <\/p>\n<h4>Canton Ticino<\/h4>\n<p><span style=\"font-weight: 400;\">In Canton Ticino (to which Locarno and Vallemaggia Districts belong), the figure of the IFeC is governed by the <\/span><b>Law on home assistance and care (LACD)<\/b><span style=\"font-weight: 400;\"> of November 30, 2010.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This law provides for the establishment of <\/span><b>Home Assistance and Care Services<\/b><span style=\"font-weight: 400;\">\u00a0(<\/span><b>SACD<\/b><span style=\"font-weight: 400;\">), which includes all the organizations (public or private) and professionals that offer home assistance and care services. This system is also known by the acronym SPITEX, which means \u201cSpital \u2013 externe Hilfe\u201d, \u201cHelp outside the hospital\u201d.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In each SACD of public interest there are various <\/span><b>Operating Units<\/b><span style=\"font-weight: 400;\">, one for each area of operation. Each Operating Unit represents a versatile team composed of many individuals, including <\/span><b>at least one designated nurse.<\/b><\/p>\n<p>&nbsp;<\/p>\n<p>[\/et_pb_accordion_item][et_pb_accordion_item title=&#8221;Who takes care of the older person that needs home support? &#8221; _builder_version=&#8221;4.9.2&#8243; _module_preset=&#8221;default&#8221; open=&#8221;off&#8221;]<\/p>\n<h4>Italy<\/h4>\n<p><span style=\"font-weight: 400;\">Home assistance for the older person is coordinated by the <\/span><b>General Practitioner, <\/b><span style=\"font-weight: 400;\">who can forward a request to the <\/span><b>Operations center for Home Care and Continuity of Care <\/b><span style=\"font-weight: 400;\">belonging to the patient\u2019s ASL. The following services can be activated, according to the need:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Planned Home Assistance <\/b><span style=\"font-weight: 400;\">(ADP)\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Home Nursing Service <\/b><span style=\"font-weight: 400;\">(SID)<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Integrated Home Assistance <\/b><span style=\"font-weight: 400;\">(ADI)<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Older persons who do not need particular treatment but have lost self-sufficiency can make a request (directly or through family members, doctors, and caregivers) to the <\/span><b>Unit of Geriatric Evaluation <\/b><span style=\"font-weight: 400;\">(UVG) to receive basic social and healthcare services. This Unit incorporates the ASL\u2019s Integrated Home Assistance (ADI) services and Services of Home Assistance (SAD) of the person\u2019s municipality of residence.<\/span><\/p>\n<h4>Canton Ticino<\/h4>\n<p><span style=\"font-weight: 400;\">In the areas covered by the Districts of Locarno and Vallemaggia, home care and assistance are managed, like in all of Switzerland, by the <\/span><b>SPITEX <\/b><span style=\"font-weight: 400;\">system. There are public Spitex (like ALVAD, partner of the REACtion project) and private Spitex.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If an older person requires home assistance, it is necessary to <\/span><b>send a report to the Spitex team leader. <\/b><span style=\"font-weight: 400;\">The report may come from family members, the family doctor, an outpatient doctor, from the hospital where the person is admitted, or from a nursing home.\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">At that point the <\/span><b>designated Spitex nurse takes on the patient<\/b><span style=\"font-weight: 400;\"> and performs preliminary research of all the health and welfare providers the person may need. It includes home <\/span><b>health <\/b><span style=\"font-weight: 400;\">assistance, from the ASL, and <\/span><b>social<\/b><span style=\"font-weight: 400;\"> services from the Municipality.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The General Practitioner activates these services upon the request of the patient, their family members, or caregivers.\u00a0<\/span><\/p>\n<p>[\/et_pb_accordion_item][et_pb_accordion_item title=&#8221;Planned Home Assistance &#8211; ADP&#8221; _builder_version=&#8221;4.9.2&#8243; _module_preset=&#8221;default&#8221; open=&#8221;off&#8221;]<\/p>\n<p><b>Planned Home Assistance<\/b><span style=\"font-weight: 400;\"> (<\/span><b>ADP<\/b><span style=\"font-weight: 400;\">) is the basic welfare service that includes the delivery of <\/span><b>medical, nursing, and\/or rehabilitative services <\/b><span style=\"font-weight: 400;\">for a period of time that is limited to disease in course. It is directed at all the people who are unable to walk and reach the clinics for needed health services. The service is activated by the General Practitioner upon the request of the patient or their family members and caregivers. <\/span><\/p>\n<p>[\/et_pb_accordion_item][et_pb_accordion_item title=&#8221;Case della Salute&#8221; _builder_version=&#8221;4.9.2&#8243; _module_preset=&#8221;default&#8221; open=&#8221;off&#8221;]<\/p>\n<p><span style=\"font-weight: 400;\">The \u201cCasa della Salute\u201d is a public structure that delivers multi-purpose health, socio-health, and social welfare services, to provide the citizen with the care and assistance that the hospitals are unable to handle.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Open 24\/7, these structures were introduced by the Minister of Health in 2007 and are present today in many Italian regions, including Piedmont.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">They perform <\/span><b>nursing, diagnostic, specialist, residential, home <\/b><span style=\"font-weight: 400;\">and <\/span><b>welfare <\/b><span style=\"font-weight: 400;\">functions through the multi-disciplinary integration of various professionals: general practitioners, specialist doctors from the main clinical fields, pediatricians, nurses, radiologists, obstetricians, physical therapists, psychologists, social workers, health workers.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">They represent a local point of reference for primary care and assistance. They cater transversally to both the <\/span><b>chronically ill patient<\/b><span style=\"font-weight: 400;\"> and the <\/span><b>acutely ill patient<\/b><span style=\"font-weight: 400;\">. They offer services to the <\/span><b>civil society<\/b><span style=\"font-weight: 400;\"> and the entire <\/span><b>community<\/b><span style=\"font-weight: 400;\"> by promoting specific prevention and health education programs.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The <\/span><b>Nurse Case Manager <\/b><span style=\"font-weight: 400;\">(ICM) plays a role of integration and coordination in the \u201cCasa della Salute\u201d.
